A System-Based, Scalable QI Initiative to Improve Care and Outcomes in Patients with T2D and Obesity

Type 2 diabetes (T2D) and obesity are 2 serious chronic conditions that are increasing in prevalence together, as more than 60% of patients with T2D are obese. Boston University Chobanian & Avedisian School of Medicine, CME Office; Clinical Care Options; Practicing Clinicians Exchange; and RealCME are piloting a customizable QI framework that can be scaled and adapted for primary care practices providing care for patients with T2D and obesity through a digital platform.
